Informed Assent for Cultural Assessment Student Questionnaire

My name is Mark Anderson and I am a student doing dissertation research in the School of Education at Claremont Graduate University (CGU). I also work as a Learning Director at Nogales High School in the Rowland Unified School District. I am asking you to participate in this study because you are a student at a public high school with a diverse student population. Your participation will take approximately 15 minutes.


PURPOSE: The purpose of this study is to analyze the cultural proficiency of teachers and school systems as a whole. Cultural proficiency is defined as the extent to which an educator of a specific cultural background effectively is able to teach, interact, and connect with students of a different culture.

PARTICIPATION: You will do the following today: Complete an online questionnaire about your opinions about the educators and school you attend in relation to understanding culture. All this should take about 15 minutes, and we will do this together in the computer lab.

RISKS AND CONFIDENTIALITY: The questionnaire does ask for some specific demographic data important to the study of culture, such as the school where you attend, gender, ethnicity, and grade level; your name will never be asked. Data will also be collected on a secure https server, and as over 200 students from your school will be responding to this questionnaire there will be no method to trace back responses to a particular individual. All responses will be confidential. As the primary researcher, Mark Anderson, is also an administrator at one of the school sites participating in the study, Nogales High School. It is important to note that the responses are for academic purposes and will not be used as a reflection of your academics or behavior at the school.

POTENTIAL BENEFITS: The benefit to participating in this research study is that your opinion about cultural proficiency will be evaluated to help your school. Through your responses about your beliefs about teachers working with students of diverse cultures, your school will better be able to identify what it needs to do to improve its cultural proficiency.
